The answer is Dengue fever. The primary vector of Dengue is Aedies Egyptii (sp). This is an urban insect requiring still, predator free water as found in old tyres, plant pots, water barrels etc hence urban not rural. The dengue virus requires a large concentrated human population in order to replicate. So while in the Brasilian metrpolii Dengue is common , in the further reaches of the Solimoes it is extremly rare.
