Does anybody know how the two different samples of DNA ( control & test ) are fluorescently labelled before being applied to a microarray chip.

I am assuming that the DNA is amplified by PCR in the presence of fluorescently labelled nucleotides but can find no evidence to back this up. Anybody know if this is correct, or can provide the correct method.

Yeah, I think it's by labeled nucleotides. But you can use any label you like. I think, that some primers bound to avidin are used as well.

primers bound to avidin are used as well

That is normally applied in Genetic Diagnosis
